All mental health approaches regardless of whether they use an outcome questioner or not are based on external observations or events. There can never be any continuity with this approach because everyone’s perception of what they are doing is different.
Example: When most women go shopping this can be a form of stress reduction. If this woman takes a man with her this can be the equivalent to a slow form torture for most men. It is the exact same experience but two completely different reactions. Notice in this example I said most. There are exceptions to every rule which makes the point I am trying to make even more emphatic.

When using an antiquated modality based on external events as are all modalities in mental health there is significant risk. Most mental health modalities use observational subjective evaluation to assess a client's progression. Recently there has been more pressure put on the mental health community to show what they are doing is working. This has prompted the use of an outcome questioner which may indicate that what they are doing is working. There is still no standard because it is difficult to establish one when one is guessing.
